- Screen Name
- E-mail address
- Phone number
- Address
- Your Partner's Full Name & e-mail address
- T-shirt size for both you and your partner (Specify: Mens, Womens sizes)
Entry fee for the Friday mini tournament is $20.
Entry fee for the Saturday Main Event is $75.
You may snail mail registation fees prior to August 28th in the form of money order or personal check (NO CURRENCY) to:
